Arriving information
Please insure that you arrive 10-15 minutes prior to your appointment time to give yourself plenty of time to fill out paper work.
If you can not make it to the appointment please give courtesy notification to the staff.
Important documents
Bring any medical records you have with you such as
- medical reports,
- operative notes
- and discharge summaries.
Bring any X-ray reports and copies of the actual test with you, such as CD’s.
Bring any labratory results.
Bring a list of all your current medications or alternatively you can fill out our online form and email it to us.
Write down all the questions you want answered by the doctor.
